Forms for fMRI participants
     It is important that we pre-screen candidates' suitability as fMRI participants. If you have been selected to participate in one of our MRI studies, please fill out all three forms below and drop them off at our laboratory, Coon 252, at least two days prior to your scheduled MRI date.
